Denied by Design: Navigating the Access to Workers’ Compensation Care in Chicago Workers’ compensation (WC) was introduced as a no-fault system to provide injured workers with timely access to medical treatment and compensation for lost wages. When the treating physicians submit a care request to the third party insurer, these requests are often subjected to a utilization review process. Utilization Review (UR) is the process of comparing an injured worker’s case to evidence-based clinical guidelines that outline national standards for a patient with similar symptoms as the one being reviewed. UR often results in a delay or denial of treatment and access to care. Analyzing these disparities can help improve patients' access to appropriate and necessary medical care. A retrospective case series was utilized to analyze existing treatment plans of multiple patients subjected to the UR process. Consent was obtained from an orthopedic private practice located in the Greater Chicago Are...

Anterior Shoulder Instability with Type IX Superior Labral Anterior to Posterior (SLAP) Lesion–about a Clinical Case| Stephy Publishers

SOJ Orthopaedics and Rehabilitation - (SOJOR)| Stephy Publishers Abstract Lesions of the superior labrum anterior to posterior (SLAP) are often associated with repetitive movements of the shoulder above the head or traumatic history. Types V to X lesions are much rarer, and their prevalence has not yet been described in the literature. In this regard, we report the clinical case of a 34-year-old man, who reported multiple episodes of anterior dislocation of the right shoulder, which he reduced on its own. The patient described onset of instability after a fall with the upper limb in extension and abduction, 2 years before presentation to the authors. An arthro-magnetic resonance was performed that showed a rupture of the upper labrum that extending through the anterior to posterior region reaching the lower edge of the glenoid, with damage to the cartilage at the anterior portion of the glenoid. Surgical treatment was proposed, with diagnostic arthroscopy and arthroscopic sutu...

Reverse Flow Sural Flap Coverage in Sequel of Electric Burn, Case Report| Stephy Publishers

Image
SOJ Orthopaedics and Rehabilitation - (SOJOR)| Stephy Publishers Abstract Reconstruction of injuries and defects in the distal third of the lower extremities represents a challenge for the plastic surgeon. The clinical case of a 46-year-old patient with a sequela of electrical burn in the distal third of the leg and the proximal third of the right foot is presented, who underwent skin coverage with a reverse flow sural flap. The treatment performed is described, as well as the surgical technique. We consider that the reverse flow sural flap is an optimal option when evaluating different forms of coverage in lesions of the distal third of the lower extremities, with positive long-term results. Keywords Electrical burn, Reconstructive surgery, Sural flap Introduction The repair of injuries and defects of the lower limb located in its distal third constitutes a challenge for reconstructive plastic surgery. The research reflected in different bibliographic sources has allo...
